CANCELATION POLICY

We ask all our patients to give us a 24 hour Business hour Cancelation , as this time is secured for you and blocked. It is unfair to other patients who really want this time and it is blocked.

There will be a charge of $100.00 for NO SHOW/ NO cancel within 24 hours of appointments

FORMS

Please Note: All forms need to be downloaded and filled out digitally or printed out.

You can bring the printed copies in with you on the day of your visit or email them ahead of time to: admin@sullivanstmed.com


*If you are filling forms out at the office, please arrive at least 15 minutes prior to your scheduled appointment time.

While we accept most major insurance plans it is important that prior to your appointment you contact your insurance company and verify that Dr. Todd McNiff is in network with your specific plan:
